Introduction
The patient selected is a 19 years old black male athlete on scholarship. Communicating with a teenager revolves around their hobbies, what they have achieved and what they would like to achieve in the future. However, during history taking, good communication skills are important to create rapport and make the teenager comfortable. In this essay, I will discuss how communication techniques affect history taking in different age groups, gender, and ethnicity. Communication Techniques
Communication is paramount in history taking. It involves the exchange of information to make a meaningful conversation. Communication techniques that enable building a health history among teenagers include welcoming the patient, introducing myself, ensuring privacy and confidentiality, active listening, maintaining eye contact, simple language, and asking open-ended questions. For a teenager, I will make the interview more interesting by allowing them to talk more about their interests and hobbies.

How might you target your questions for building a health history based on the patient’s social determinants of health? Develop at least five targeted questions you would ask your selected patient to assess his or her health risks and begin building a health history. Write a summary of the interview and a description of the communication techniques you would use with your assigned patient. Explain why you would use these techniques. Identify the risk assessment instrument you selected, and justify why it would be applicable to the selected patient.
