The proposed change is the creation of a cancer care center at Upper Connecticut Valley Hospital (UCVH). This occurred through determining that the facility offers comprehensive health care but does not offer cancer care and yet cancer patients exist within its community. The second stage entailed identifying the right people with the right mix of commitment, knowledge and skills to address the urgency identified in the first stage. The third stage entails creating the correct vision that takes into account objectives and goals. The vision identified for the proposed change is to provide comprehensive cancer care services. The fourth stage is communicating with stakeholders concerning the change. The fifth stage is empowering action by getting the team identified in the second stage to create the cancer care department, getting support for the change, eliminating roadblocks and constructively implementing feedback. The sixth stage is reducing pressure through focusing on short term goals as achievable aspects of the long term goals. The seventh stage is persistently following through with the change. The final stage is incorporating the change through reinforcing it by making it a part of UCVH identity. Applying this model is expected to offer two benefits that presented as a progressive approach with easy to follow steps, and focus on the change results rather than the process. Still, the model is expected to present two shortcomings presented as rigid steps to be followed, and following the model can be time consuming (Anastasia, 2015).
