Digoxin
According to the American college of cardiology foundation, digoxin is the drug of choice in treatment of heart conditions such as atrial fibrillations and heart failure with reduced ejection fraction (Lam, et al, 2020). The dosage is initiated at 0.0625mg and maintained at 0.125mg daily. Its bioavailability is 60-80% and the onset of its effects is up to 2hours. Mode Of Action
Digoxin exerts its positive inotropic events that promote myocardial contractility. The inotropic effects occur through the inhibition of the sodium-potassium pump at the cellular membrane. This causes an increase in intracellular sodium concentration, a decrease in potassium, and an increase in calcium at the cytoplasmic (Lam, et al, 2020). This will increase heart contractility and cardiac output. Digoxin has vogomimetic effects that inhibit the atrial ventricular node by stimulating the parasympathetic nervous system which slows the electrical conduction and hence a decrease in heart rate.
Adverse Effects And Monitoring
Digoxin can cause life-threatening and mild adverse effects after administration. Digoxin toxicity is the major cause of fatal cardiac arrhythmia (David, & Shetty, 2020). Other effects of digoxin are visual effects, tachycardia, vomiting, and anorexia. Before administration of digoxin, the nurse should ask about any recent change of medication to avoid drug toxicity. One should also evaluate the glomerular filtration rate, renal function, and regular electrocardiograms.
Drug Interaction
Digoxin cannot be administered along-side antibiotics such as azithromycin and clarithromycin because they increase the effect of digoxin by altering the intestinal flora (David, & Shetty, 2020).
