Briefly explain your search strategy

A search strategy describes a structure that is clearly organized for the use in conducting database searches using key terms. It includes the key concepts related to the research question and assists in the retrieval of timely and accurate results. In this case, the analysis of current evidence based practice guidelines for the treatment of otitis media combined the use of phrases and key words combined by AND, NOT and OR modifiers. Who developed the guideline?

The search strategy I employed was developed by George Boole and is referred to as the Boolean search guideline strategy. The logician, philosopher and self-taught English mathematician was born in 1815 and died at the age of 49 in 1864 in England (Bachchhav, 2016). He was a shopkeeper’s son who had acquired a low level of education. Boole was tutored privately by his father, learnt Greek and Latin as well as mastered some English by the age of 12 years. He practiced clerical roles and later opened his school at the age of 20 where he tutored mathematics and later became a professor at the Queens College (Bachchhav, 2016). His study of algebra resulted in the development of his mathematical theories of logic and probabilities which is significantly applied in the computing and research field.

The Boole search strategy is an original search strategy developed and published in 1854 by George Boole. Boole developed this strategy to narrow down the search results into a more relevant subset. He considered the use of AND, NOT and OR modifiers as an effective way to conduct a search. The search eliminates unwanted results and allows only the most relevant and specific concepts. How the conflict of interest was managed

The conflict of interest in this guideline was managed through the use of specific modifiers that combined phrases and narrowed down the search to specific concepts and results. Most of databases support the Boolean search which allows the use of Boolean operators in searches. The strategy uses AND modifier as an interception operator while OR is employed as a union operator. On the other hand, NOT is employed as a compliment that identifies the opposing information.

How is quality of evidence defined?

The quality of evidence is defined as the confidence of the effects of the estimates reported and their adequacy in supporting specific recommendations (Djulbegovic et al., 2015). The type of evidence is ranked as high, moderate and low quality. High quality evidence gives strong recommendations and therefore, further research is not likely to affect the estimates. On the contrary, low equality evidence provides little confidence in the support of recommendations and thus, the recommendations are likely to be affected by further research. Consequently, moderate quality of evidence gives moderate confidence in support of the recommendations and hence changes may or may not occur from further research.

Explain the differences

Recommendations describe proposals or suggestions regarded as the best course of actions supported by a body of evidence. It can be a statement suitable for a particular purpose or bearing something that should be done. Strong recommendations are courses of actions suggested for a particular project and are made from high quality of evidence. Notably, options are courses of action that may or may not be chosen. It keeps an open offer that can be withdrawn or conducted. As such, it is a choice made out of multiple possibilities.

What are “key Action statements?”

      Key Action statements are the explanations and algorithms for the care process, treatment, evaluation, diagnosis and monitoring among other services. They promote best practices and are guided by opportunities. They seek to minimize care variations, reduce resource utilization and inappropriate care. The evidence supporting these decisions is systematically reviewed.

The specific treatment recommendations

Specific treatment recommendations for the patient entail a diagnosis through the otomicroscopic or otoscopic examination of the tympanic membrane (Thomas et al., 2014). The diagnosis for otitis media in pediatrics is characterized by the presence of moderate to severe bulging of the tympanic membrane or mild bulging with reddened membrane or earache. The specific medications for the patient include ibuprofen and paracetamol. Antibiotics such as amoxicillin are given in the dosage of 50mg/kg divided into three times a day. The follow up plan for the patient is set to be done after 3 days to monitor the patient’s progress. This will then be extended to 2 weeks and finally 4 weeks intervals to 3 months. Referral of the patient will be done when the treatment options provided present adverse side effects or when the patient fails to show any improvement. The worsening of the symptoms after medication will also necessitate referrals for further examination of the underlying cause of illness (Chonmaitree et al., 2016). Prevention of otitis media will be done through adequate influenza and pneumonia vaccination, avoidance of tobacco smoke exposure, use of xylitol lozenges during the cold seasons and reduction of upper respiratory tract infections in children (Norhayati & Azman, 2017). Pain management is possible through the administration of paracetamol and ibuprofen analgesics.
