Interprofessional collaboration in the inpatient psychiatry is important in improving the quality of care delivered to patients. It facilitates planning during transitions and thus my facility promotes interprofessional collaboration through encouraging full participation of practitioners in the transition process. This involves attendance of training inn other departments to get insights regarding the hospital roles and enhance effective communication among the team members thus promoting interdisciplinary rounds. Moreover, the organization promotes focus on the patient which guides different experts in achieving positive patient experience and satisfaction.

The role of the nurse is to participate fully in the patient transition process. This involves the completion of the transfer using evidence-based practices that positive outcomes as well as performance of nursing assessments to maintain care continuity. Moreover, nurses are involved in the management of the transfers through effective communication with patients and interprofessional teams to promote participation and prevent the occurrence of errors. They also consider comprehensive care models which make the transitions more effective while at the same time keeping the patient’s welfare in mind. The gaps identified in the transition process include patient knowledge. These are experienced during and after discharge and are caused by forgetfulness among the physicians. For instance, the physician at our hospital has forgotten to obtain preapproval from the insurance company on various occasions for the necessary discharge medications. This has limited the access of medication to patients thus undermining the quality of care provided by the facility. It has also resulted in negative feedback delivered through angry phone calls from the patients and their families complaining of denied access of drugs at the pharmacy.
