Background

During a night shift, healthcare workers in the Emergency Department (ED) were exposed to unknown chemical substances from a patient. For healthcare worker safety, supervisors called a code orange (hazardous material/chemical spill incident). Per hospital protocol for such an incident, all staff within the area of exposure were to report to the outdoor decontamination tents located directly behind the ED (about 10 people were affected). Hospital policy included having the city fire department respond to the facility and assist with decontamination. The staff members were directed to remove all clothing to be decontaminated by a fireman in a hazmat suit. After decontamination, the staff members were directed to exit the tent unclothed, in front of other hospital staff, to receive hospital gowns and towels. Many of the staff members who were exposed to the chemicals were women of Muslim beliefs which prohibited them from being seen naked in front of a man. Islam is a unique religious group in the world. In fact, the Muslim faith encompasses several ethnicities with diverse views that influence decision-making, workplace dynamics, health practices, risks and use of health care. The Muslims ascribe to an Islamic faith that does not embrace a Marxist/socialist or liberal worldview. Instead, the Islamic faith embraces a unique Islamic worldview that is based on monolithic interpretations of religious texts. The source of the religious guidance are embodied in the Qur’an (Islamic Holy Book) and Hadith/Sunnah (Prophet Mohammad’s teachings and legacy including his deeds, acts and words). This implies that for any Muslim, the actions in the workplace must not be contradictory to the way they understand their faith. Should the workplace fail to harmonize between the Islamic worldview of their Muslim staff members, then these staff members are likely to encounter some form of social exclusion within the larger Muslim community with the mismatch harming them via adverse perceptions (Abdalla, Chown & Abdullah, 2018).

As a result, the workplace must have measures in place to accommodate these differences and approach the needs of the Muslim staff members through an Islamic lens while noting that the different workplace policy and protocol requirements are attributed to cultural and societal practices.

Significance

There was a delay in the decontamination of these staff members prolonging their possible exposure to an unknown chemical. Unfortunately, aspects of cultural importance were not initially considered or addressed. A code orange is a very uncommon occurrence in the ED however, this situation forced an examination into the policy that was developed decades ago and outdated. Development of a new policy that is culturally competent is essential to not only ensure inclusion but also patient safety. After recognizing a gap in culturally competent care, opportunity for education and immediate change in protocol was identified. In the year since this situation, it is important to assess the impact of such care on the healthcare worker during and after the decontamination period. Research Question

How do healthcare employees involved in the decontamination during a Code Orange, perceive the event related to health and cultural beliefs post decontamination intervention?

Audience

The audience is identified in two groups. First, facility administrators who have the power and authority to directly influence workplace policy and protocol development through determining resource allocations. Second, providers who are directly affected by the workplace policies and protocols, and can offer insight into how the changes have affected them and if there are opportunities for improvements (Grove& Gray, 2022).

Who is most likely to use this research? 

This research is likely to be used by health care facilities that employ personnel from diverse cultural and ethnic backgrounds. This research results would inform them about the need to account for diversity within the workplace, and help them in creating a workplace environment that supports all personnel with different worldviews (Grove& Gray, 2022).

What is the practice setting?

The practice setting is a hospital facility, particularly they ED. In this case, it is noted that Code Orange protocols require that when facing an unknown chemical contamination, the medical personnel exposed to the contaminant should go through decontamination protocols as soon as possible. This includes discarding all the clothes and going through a decontamination shower/bath. This implies that some level of nudity is involved thereby creating a concern for staff members whose worldview does not allow for public nudity.

End-user needs: How should you present data to make it most usable?

The needs of the end-users is to be presented with a Code Orange protocol that accommodated their unique worldview and diversity. This means that the revised protocol should not expose Muslim staff members to public nudity during decontamination. This consideration should extend to other unique cultural groups that may be uncomfortable with aspects of the Code Orange protocol.

Data presentation will follow a five-step approach to make it most usable. First, ensuring that the data can be seen and is readable. It should not have hard to see labels and numbers. Second, focusing on the points the data illustrates. The presentation should be about the meaning of the data to allow the audience process the information and buy into the argument being presented. Third, each data visualization should only share the major points that significantly support the argument being presented. Including many points in a single visualization would only confuse the audience. Fourth, clearly labeling the data components and avoiding abbreviations that are not obvious with a focus on visual intuitiveness and clarity. These conferences offer a learning facet that exposes the audience to new ways of operating and can help them discover new ways of being more productive (Schmidt & Brown, 2019).

As future nursing practitioners, it is important for you to understand that the results of evidence-based research practice should be written for others to understand and apply to their practice. Research needs to be disseminated as widely as possible at all levels, both national and international.
