Patient’s History And Medical Information

The patient is J.A 36-year old white American female attending therapeutic sessions for depression. Recently, She presented at the outpatient department with sleep disturbance, loss of appetite, and generalized body weakness. She reported to the doctor that she feels a lot of hatred for her children, she feels self-unworthy, and has had suicidal ideations.  She reports that the onset of these symptoms was after her husband divorced her after 16 years of marriage. She lost her job as a call center manager due to anger outbursts to her office workmates and poor work ethics. Currently, she is living with her parents and her two children. She takes spirits, smokes tobacco, and marijuana when she feels overwhelmed. Her mental assessment reports of neat looking appearance, maintaining eye contact, speech tone and volume was normal. The thought process was congruent, with no hallucinations and suicidal ideations. She is currently on fluoxetine 40mg monthly dose.

Patient’s Diagnosis And Justification

Depression is a psychiatric illness that presents with persistent sadness and loss of interest. According to the DSM-5 criteria of diagnosing mental health illnesses, patients with depression present with reduced pleasure or interest, sleep disturbances, suicidal ideations, fatigue, impaired concentration, and thoughts of self-worthlessness (Tundo, et al, 2020). J.A presented with almost similar symptoms at the outpatient department; feeling unworthy, suicidal ideation, sleep disturbance, fatigue, and reduced appetite. The patient’s symptoms meet the DSM-5 criteria hence, the diagnosis is depression.

The Most Effective Psychotherapeutic Approach With The Family Expected Outcome

Both solution-based and cognitive-behavioral therapies are effective and efficient approaches in the management of mental health disorders. Solution-based therapy aims are solving the problem at the family and social interactive level to improve the quality of life and decrease distress (Mehrabi, et al, 2020). This would best apply to the patient to solve the underlying issues with his family and a place of work.

Cognitive-behavioral therapy aims at restructuring the patient’s thinking and behavior (Brantnell, et al, 2020). It will be effective for this patient since it will help in changing the suicidal thoughts, feeling worthless, and hatred for her children. It will help the patient to avoid indulging in alcohol, marijuana, and tobacco in cases of emotional outbursts. Ethical Considerations For Each Patient

Ethical principles are essential when handling patients with mental health disorders. Before initiating any form of treatment, the care provider should seek a fully informed patient. The nurse should have the best interest in patient management and protect the patient from harm in cases where they have to make decisions for the patient. The nurse needs to keep the patient’s personal information private and confidential to avoid stigma in society (Chenneville, & Gabbidon,  2020). Most psychiatric patients tend to go against the advised method of treatment which makes the nurse violate the principle of autonomy.
